Appointments: Office hours are from 8:30am to 5:00pm Monday through Thursday and from 8:30 to 12:00 on Friday. On your first visit please arrive 15 minutes early to allow for the initial registration procedures. If you must reschedule an appointment please notify the office at least 24 hours prior to your appointment so that we may schedule another patient.

Fees: Efforts are made to keep the cost of services down. The full fee is due at the time of service. We will gladly provide you with a receipt you may use with to seek reimbursement from your insurance company.

Emergencies: We request that you call during business hours for normal matters, such as appointment scheduling/rescheduling and medication refills. In case of an after hour emergency, the answering service will page the clinician on-call.
